    Late reply, but Quality Consortium Services/Quality Physicals. He’s out of MN. $49 a year for the subscription then the tests are charged separately, I think $59 a pop. Just tell him what area you’re in and he’ll find a provider to use. Usually responds within an hour to emails regardless of time of day. Great customer service
