The biggest difference, when looking only at the cab, is the cab width.
The euro cab looks a lot higher than the american, but the cab floor is also a lot higher. The real hieight of the cab itself is not that different when comparing low cab roof to low cab roof (BTW, the Scania in the picture already has the higher roof).
If you remove all the plastic below the cab you will get a better image of the cab itself.
I think @98989 posted a picture of a Mercedes cab without all the plastic. It clearly shows how high the cab floor is above the truck frame.
This pictures shows a medium roof DAF, with part of the plastic (below the door) missing. It gives some indication about cab floor height.
Now, if you have the high roof cab, for example DAF SuperSpacecab, the roof height is not much different from an American high roof sleeper.
The lower floor height of the american cab however results in higher interior space.

I added the red arrow to indicate floor to roof distance, blue to indicate engine tunnel to roof.
EDIT: engine tunnel gets a bit higher in the rear of the cab, so blue arrow is not accurate for middle of the cab,

It would be a midroof.
For those who don't know but that Scania i posted is a model from the late 80's,early 90's.
There are very few pictures that i could find with a Euro truck next to a US hood.
And you cannot use Australian pictures as the hoods there are specific to their market.Bean Jr., spsauerland, KVB and 1 other person Thank this. -
They would have to install the cab higher to clear the engine if they wanted a true flat floor.
Now (for our north american friends), if you look a the picture below, this is more or less what you get in DAF's largest low roof cab (it is not really a low cab, but the part in the picture is without the higher roof) just to compare to american low roof cabs. The biggest difference is the width of the cab

Sure, not many buy a flat roof truck, but some do.
The ones who do are usually small companies or owner operators who like old school. Just like north americans buying small cab PB's or KW's? We're not that different.....

Can't say for other brands but with Daf the flattop is more expensive then the midroof.
Reason is the very low numbers.
Production is so geared for the midroof and highroof that a flattop is a special order truck.
