High hook, incomplete hook, or former driver pulled the pin.

So.. something overlooked and/or operator error most likely. No tug test or visual after a hook. I thought if you do a high hook you would not be able to pull the trailer...if the shank is not in the opening then the jaws won't shut, will they?
I've been told about drivers pulling the handle but if that's a possible reason it's still not a likely explanation, is it?

A high hook is quite hard to uncouple. Itll pass a tug test or 3. You need to look and tug.

Being western express it's almost guaranteed driver error.

I wish I could see what that looks like. I can't figure out how it's low enough to get hung up in the jaws but not fully seated.
Is there a video anywhere showing what it looks like from underneath?

If the lock bolt is seated it's a pretty sure thing. Don't worry too much.

I was told once that if you back into it and give 2 tugs that some? All? Jaws have like notching that like locks the mating surfaces of the jaws do they can't slip outwards. Is that a real thing?

Also. Must worry too much. That's my purpose in life apparently LoL
