Perhaps it is. Totally missing on 2 injectors. I know for sure the middle o-ring isn't supposed to be cracked or broken.

Yes the o-ring on the tip is designed to burn off and be replaced by a dam of carbon.. but yes obviously the rest of the o-rings should be in tact

UPDATE: Replaced all injector orings and hey look same problem... pulled the exhaust manifold back (one broken stud). Fired it up and there's 2 wet exhaust holes #2 and #4. Will be swapping 2 & 4 injectors to other holes to see if the problem follows. Probably should have done that first but it needed orings so... if you have a similar problem just pull the exhaust manifold back and see what you have. Plug the oil supply line for the turbo so you can start it. I used a piece of steel plate with a bit of ultra black sealant and some vice grips to hold the plate with no leaks.

Ok, I actually have 3 exhaust holes (2,3,4)with fuel and white smoke. Unplug the injector while running and smoke goes away on all 3. Swapped #1 & #2 injectors and problem didn't move to #1 cylinder. Looking at options now. Going to air up cylinders one at a time to see if air is leaking out valves or rings. Not sure if there is a way to check ecm power. Injector harness in rocker is brand new. Have to check fuel pressure as well just in case. Any other ideas or if anyone has experience on compression test or leak down test would be nice.

Your injector seats in the head, are bad. I can machine them, in-frame.

How do I test to verify injector seats

Smoke on start up. O-rings burned up. Carbon build up on injector barrels. Air in fuel system.

Put the fuel return line, from the head, into a bucket of diesel fuel. If bubbles come through, your injector seats are most likely bad. I repaired a lot of them, with your exact symptoms.
