good equipment, does not even have to be new, just very well maintained.
choice of working days/nights, rather than being told i MUST work x number of days or nights
all tolls paid with EZ pass
"truck money" for odds and ends like washer fluid, paper towels, rags, etc.
office staff that will not force dispatch, but rather "ask" if i can do some extra work.
direct deposit
retirement plan choices, with matching company contributions, "up to x amount weekly".
at the very least 4 weeks fully paid vacation, after at least 1 year of employment.

Pay is important as well obviously. I should say if pay is the most important thing to you, you should probably just go LTL linehaul extraboard (until you get enough seniority to bid on a high mile run). On a side note Americans have a horrible work life balance compared to most other 1st world countries. Most people in 1st world countries get about 4 weeks of paid vacation per year regardless of job or sector. So janitors, blue collared, white collared, they all get 4 weeks mandatory paid vacation. The US is terrible when it comes to work life balance, check out this chart for reference to see how pathetic the US is in this regard:
