    thank you for replying . the name on my truck and the cab card has the company name. the title has my name on it

    Same. I drove around for months without it, and happened upon the requirement just browsing the Internet. I called OOIDA, and they knew exactly what I was talking about. They emailed me a generic commercial lease agreement, took me ten minutes to fill out, and now I just keep it in my truck book.

    Exactly how I have mine set up. I put the truck in my name when I bought it, because I didn't know any better at the time.

    I searched OOIDA and could not find the lease agreement. I need one so I have it in my truck. Do you have a link?

    i called ooida an talked to their compliance people and they emailed me two diff. forms.
