The only two things that usually cause that issue is a thermostat or water pump. The lower radiator hose should be a little cooler than the top, but if it's cold, and the upper is very hot, the coolant isn't circulating.

I'd change the thermostats, and if that doesn't work, then change the water pump. Due to the age of the truck, it may be a good idea to just change both the water pump and thermostats right off the bat.

I'd just wash out that radiator. Your cooling fins are probably all plugged up with dirt and bugs.

if cleaning the radiator doesn't do it, there are two thermostats that could need changed. They aren't that expensive. When I had a detroit, my thermostats hung open so I couldn't build heat running down the road. Once I changed them, it could be -20 or 120 and everything worked like it should.

Sounds like the thermostat, and thats probably where I'd start seeing is its a very cheap part, I believe there is two of them on a detroit but I'm not very familiar with detroits.

Yes, on the series 60 there are two thermostates located in a housing that your delta p sensor sits on top of.

He doesn't have a delta p sensor on a 99. Thermostats are in a housing on top of the head, midway on passenger side.

What temp. is the fan kicking on at? It should be on before 220 it may not have enough time to start cooling before it reaches shut off temp, CK. behind your A/C con. dirt can be piled up in between rad. an charge air cooler stopping air from getting to rad.
