The four major players come to mind Blue Cross, Blue Shield, Anthem Blue Cross, United Healthcare.
yes the insurance sales agent can possibly give you a discounted rate but that's few and far between. remember the exchanges give you a deeper discount because of the volume.
Plus you need to check out what's available in your area. some insurance companies will not cover you depending on your location.

for example if you had Kaiser HMO in California. Kaiser I believe is in 10-15 states. Now if you go out of network then you need to call a special number to get that medical coverage taken care of.
then if you have a PPO then you can pretty much go to any medical facility and get taken care of, but please know that you might have to pay a higher deductible and out of pocket cost.Last edited: Oct 4, 2015

Crossing state lines was NEVER ever part of it. Nor was it supposed to be. Reason every STATE mandates what coverage an insurance company must provide. Crossing state lines blurrs things. As a lot of people would only shop by price. Then when something happens they would file bankruptcy on the health care providers in thier state.
A real life example. Back in 95 i was paying $900 a month for family coverage in mass. But that was for health, eyes,dental, and everything was covered.
We moved to indiana for 3 years. When we went to get insurance thier. It was only $400. BUT cancer wasnt covered,that was a seperate policy, pregnancy wasnt covered again seperate policy. Blood wasnt covered. No out patient (clinics) Same with eyes and dental. The co pay was also higher. So what at first seemed like a good plan with a nice saving. After adding in the seperate policys cost MORE for less coverage.Last edited: Oct 21, 2016

I am not so sure crossing state lines will do much anymore. There are only a few providers left, and the smaller ones that are left have a very limited area as they do not have the resources to offer care outside the area they operate in. If you want to know why costs are increasing, look at the last 15 years, most insurers were non-profit, now the majority are for profit and responsible to shareholders.

In the final analysis, Obamacare is doing exactly as it was intended... to screw up the system enough so that eventually everything will have to be scrapped and the only thing going is a government health care system like other countries.
