Answers the questions HONESTLY on the Epworth sleep scale online.

According to my doctor, that is the first test he administers BEFORE he recommends or prescribes a sleep study itself. Big neck or not.

17" ( FAT ) neck, and BMI over 25 you're going to study period.
    17" ( FAT ) neck, and you're over 50, you're going to study period.
    Those are the rules.
    I have 16-1/2" ( MUSCLE ) neck, 52, no sleep study, 2 year card.

No one answered my specific question, but I guess I can infer that even if he brings his current CPAP report, he’ll be sent for a new sleep study regardless.

WRONG! One size does not fit all.

I do my DOT physical at the facility where my primary doctor is located. The DOT doctor in their system has access to my medical files. My primary doctor has included in my medical file that he has administered the Epworth Scale test on me and that I don't have sleep apnea.

Even though I meet the above listed criteria for sleep apnea test. The DOT registered doctor says nothing to me about it ;-)

I object to these poor guys who are sent to some overnight "sleep study" facility, hooked up with tubes, wires & what not, in a strange place and then told to go to sleep.

    Whatever, man.

In addition, I think if someone really has sleep apnea, they pretty much know already something is up. If I thought I had it and being on a CPAP machine would include my quality of life, I would have NO problem being on one, period.

One size doesn't fit all, man, but those are the rules, man.
I didn't write them, man. But they are what they are, man.

Schneider DOT physicals are as close as you're going to get to a cattle walk.
Now you go someplace that's a teamster shop, you better be in shape to get a 2 year card.

Hey, man, where does it say that it's mandatory if you meet the criteria, man? I was under the impression it was something the DOT doc could require, man. Not that it's automatically required, man.

All kidding aside. I do realize that some companies may have stricter standards that they can impose on pre-hires. However, I don't believe the criteria you listed Dave is an automatic requirement for sleep apnea.
