On the exhaust manifold yes/no....if you assemble using the CAT centering dowels that fit into the counter-bored holes on each section and use proper torque and sequence most of the time that eliminates a lot of issues. The VVA studs were a known issue and by now I'm sure they've all been replaced with updated studs. But, without disassembling the overhead you won't know for sure. 1 million miles and I would be checking bushings in the front gear-train, especially the Cluster Gear (think DDEC Bull Gear). You can take the serial number into CAT and see what warranty work had been done along with other repairs if done by a CAT certified shop. They're strong engines, not the most fuel efficient but proved pretty reliable in their day.
