So if I buy a truck they still has a factory warranty, let's say I break down many states away from the state I bought the truck. I get it repaired, do I pay cash and the warranty reimburses me with a check in the mail or do they confirm they warranty covers the cost upfront before the repair and there is no charge?

You have to take it to a authorized warranty service center (read dealer), and they will confirm via computer the warranty status and make the repair.

Anything under warranty is billed to warranty. The rest is billed to owner.

And in three weeks they may get to you... Warranties in most cases aren't worth spit, but you will learn. ;-)

Plus, depending on where you breakdown, the tow fees will outweigh the cost of someone close doing the work.

There are some used trucks that have towing included in the factory warranty (the original purchaser went all out). I almost bought one today. But they are rare.

If it's the OEM warranty and you break down and the failure is warrantable you won't pay anything at all.

The aftermarket warranties have towing in them too.

It's part of the basic coverage. Because some items require IMMEDIATE shut down, or you'll lose the engine.

Think mine pays $1,000 in towing.

Your question has a HUGE range.
