well if I picture is worth a thousand words, here is two.
1st photo is the new style, no pillars, solid rubber
2nd photo is the worn out old style. notice the rubber pillars are cracked and some are broken loose

A brand new "old style" will look kinda of worn out at the bushings. If the rubber looks fine(not unlike the one above), take out the two bolts and drop the driveshaft onto a jack stand. Spin the bearing, if it clicks it's shot.

It's not super difficult to change out. Just make sure that you visually double check that the yoke is in phase upon reassembly. If it's out even one spline tooth, you can run the risk of severe vibration and loosing the U-joint. Make sure it's 100% lined up right.

Usually just one. I feel like I may have seen two before, but I could be mistaken.
Not terribly difficult to do, if you have the right tools to do the job. -
