Where do you suggest I look next? We tested pressures on fuel system, ran injector cut out, fresh filters fuel and air as well as feed lines. Would a barometric pressure sensor be a possibility?

Why was it programmed for Low NOX right before you bought it?
Sometimes you gotta quit guessing and actually go look around inside. I'd personally start by looking for a loose injector/bad injector O rings/injector cup issue.
I've seen too many issues there and it's often overlooked, cause of course, "It couldn't be that"
Did you buy this from a dealer?
BTW, Don't be the guy asking for help and then just disregarding all the feedback you get. I'm with @wore out here, and this programming has nothing to do with your issue. -
It was already suggested to test your oil to see if you have fuel dilution, if you haven't already. I had a completely inframed S60 that started smoking with the smell of diesel right away. And it turned out to be a bad newly rebuilt injector tip. Is it possible that fuel leakage is intemitant, or small enough that an injector cutout test might not pick up?

It was purchased privately and flashed to a low NOX prior when previous owner took it to a dealer for an overhead and some other work last year.
It ended up being the BOI timing. Running fine now.
I did not disregard "all the feedback I recieved. I asked about tuning.
Thanks.jaffles and Rideandrepair Thank this. -
We ended up solving it with turning. Oil samples are clean. Thank you for the reply.

It doesnt smoke in cold weather anymore than any of our other rigs with other PowerPlants. The issue was very intermittent and only during light to moderate acceleration after coasting or low speed travel, usually in town. Issue only showed up after gaining elevation.
Ended up advancing some parameters and playing with some tuning and its right as rain. Thank you for the replyjaffles, Rideandrepair and Magoo1968 Thank this. -
2002 should have already had a lownox program since new. Everything after Jan 1 2000 per EPA settlement. Never heard of BOI timing. Can you elaborate? The symptoms are a common problem. My Truck does this occasionally, after an inframe. It’s a late 99 engine build. I suspect it came new with a lownox program. This is the 2nd inframe. It was fine till this inframe. Only thing different is a new non wastegated B&G turbo. That was a mistake. Maybe related, IDK. I’ve yet to hear of anyone figuring out the problem, except for the vague “ adding a tune”. Any idea what BOI stands for?
