I've got a 2007 Western Star with a Detroit 515 in it. Putting antifreeze in it every few days. No leaks have been found. Do have some white smoke and a bit of loss in power. Oil cooler replaced and pressure tested, no leaks. There's a slight antifreeze smell when I turn on the bunk heater.

Any additional smoke when you start it after siting for a while? I had an EGR cooler start leaking a few years ago, white smoke on start up, burned about half a gallon of coolant a day heading back to the shop.

Yes but just checked the oil and it's sludge. Could that be head gasket?
Hoping I won't need an overhaul. -
There is a few things that it could be but a shop will be able to tell you for sure and not be guessing like we would be not being able to look at it ourselves

I am wondering about your fuel mileage. I do found a 2006 to buy, 18 speed, 515 HP also.
Also, is it ok to find loads , as O/O, with that truck age?
