Can your employer require you to supply copies of your CDL Physical "long form"? I know you have to give them the copy of your Medical Certificate card per DOT regs, but the complete long form? Does this violate Hippa? I don't recall anywhere over the years any memos from the employer requiring this or are the lower level managers mis-informed?

Yes they sure can. Those are kept in your employee file. My employer required my medical long form upon hire and when I go to get a new physical done I send them the long form. Some companies require it and others just the card.

It's funny you ask that.The carrier i'm going with ask for mine today!!!I had my school email me the attachment and just printed it off,so i'm good to go!!!

DOT ask you for the Medical Card, you better have the long form too with Ins. Reg. Etc.

Only if you want to to give them one.
The employer only gets a copy of the certificate.
Unless you signed a release for your medical long form.

take it people forgot about the new medical certification process. Pretty much doing away with the old way.

It started last year. I am already self-certified and am no longer required to carry the medical card. State already has it in the system. Full compliance is to be completed by Jan 2014. If you haven't done it yet, it's just a matter of time on this one.
