You CANNOT be afraid of heights. Melton in particular will have you walk to the edge of a couple of stacked containers in order to demonstrate this.

Plenty of small Companies that advertise already having a dot physical. The downside is they don’t pay for it. The way certification works now, Dr. files straight with feds, or through the state, however it’s done. IDK exactly. The result is the two companies I’ve worked for could care less. They don’t even see or ask for a copy. I’ve never been asked for a long form copy. My experience is limited. For 20 yrs I was Independent. I’m on a 3 mo. now. Going for another in 3 weeks. Hoping to get a 1 yr. The Company I drive for is clueless as far as I can tell. My advice is to forget about having any mental health issues. From now on you’re fine. Doesn’t matter who diagnosed you with what in the past. Things often change. Stop being so paranoid, you’re making me nervous. Lol. Go for it. Seriously, try not to negatively talk yourself out of opportunities. Half of society could probably benefit from some pill. A huge portion are taking some form of happy pill. Best to keep that to yourself. I wish you the best of luck. Once you get rolling, you’ll see. Companies just want to be compliant with the regs. They don’t care about the details.

Many, by far not all, training companies will screen drivers by sending them to DOT physical of the company's choosing.

The big boy companies, by that I mean companies that hire adults, not diaper wearing trainees, don't really care where you got you physical.

As you learn this company characteristic, you can use it to screen them, before they screen you. Do you want a nanny or paycheck?
