Possibly Quantity Control Valve in the fuel pump.

Early signs of a defective one is surging at idle and smooths out with a little throttle.

Its job is to meter the amount of fuel entering the High Pressure Pump elements.

Other cause could be plugged ports in EGR venturi.

My 2010 Western Star with a DD15 is doing some what the same except it runs better when the engine is cold. When the engine warms up and I am at a light and start giving it fuel to go it falls dead on it’s face, I have to try to rev the engine up and then it will move. Also when I do it will send grayish black smoke out then it runs fine. Going down the highway it runs great I hate to stop because I know it will act up again.

Did you figure out the issue. I’m having the same issue with mine. But mine starts to smoke when water temp reaches 150. I give gas and it stumbles at 800-900 rpm and white smoke from unburned fuel comes out then clears up once rpms get higher. It also smokes on idle sometimes. But most of the smoke is at that specific rpm

Did you find problem? Same problem for my 2016 dd15. White smoke at 900 -1000rpm. Replaced injectors, ECM nothing helps.

    77fib77 Thanks this.

No it does it some times and other times it doesn't. I think it might be the actuator for the egr valve. I want to get the computer program and fix it my self. I don't want to that until after Christmas. Make money while the rates are up. It light be the valve too. But dont know for sure.

Just throwing parts at it. I have changed the delta P sensor.

    Is your truck deleted?

Change the EGR venturi with the sensor. its the pipe the sensor mounts to. The ports plug up and cause all kinds of issues with no fault codes. Also the sensor needs to be calibrated with dddl after replacement. Also, a lot of the fueling is programmed to this sensors readings.
