2. Date of shipment is the date you picked it up regardless if it was sitting on a yard.
8. They ain't going to get that detailed. Unless it's a dedicated account, it usually includes billing after the fact. I would put COD as most have.
9. Is open to whatever you decide. It can be pc (pieces), cs (cases), bn (bins), ea (each) or the easiest, pallets.
11. You nicely explain to them the law requires you to carry a BOL. Show them the regulation if they argue. They either produce the original or comply with the handwritten. I've never had a customer give me a hard time with a hand written.
No company can reprimand you for not breaking the law. If you do get stuck in a position with a bullheaded butthole, then create the ficticious bill, haul it, and raise holy hell on Monday with your dispatcher and safety. I guarantee you the weekend dispatcher is going to hear something. Odds are very slim that particular load is the load they check everything. If anything, they'll look at the dummy bill and send you on your way.
Things like this happen occasionally. You just try to deal with it smoothly with the least headaches and try to make everyone happy, even if it means being sneaky.airforcetoo Thanks this. -
Create a BOL to the best of your ability with the information that you have, have a good trip and don't stress over it. Part 375 applies only to the carriers of household goods.

Well, First of all thanks to everybody for your help; The problem has finally been resolved. I wrote an educated and detailed qualcomm message to the weekend dispatch guys requesting either they provide me with the missing information or get me off that load. I quoted FMCS reg 375 and I guess they got scared and answered right away with the info. Stoped at the 1st truck stop got some blank BOLs and now I'm half way there. This was a good learning experience though.

I'm laughing because this is being way over complicated.
Simply write out a BOL with all the info you can put on it. I would be shocked if some officer says "wait wait.....oh no, you do not have criteria #11 out of 14!"
I have had hand written BOL's that said what the product is, where it's going and where it came from, and was fine.

They only care about customs documents and declarations, a bol means little if anything to customs.

Right, so as long as you're not crossing a border, all you need is a piece of paper with a few brief pieces of information.

Thanks, I'm just new to this trucking industry man and I'm just trying to watch out for my bread and butter, in my previous industries you really had to be detail oriented cause if you weren't paying attention to the minimal detail that was your job. SO yeah guys sorry for all the paranoia, just trying to do everything the right way.mickeyrat Thanks this.

I'm NOT trying to make it so you don't pay attention to details, but having a complete "14 point" BOL for everything I have on my trucks is not at the top of my list of things to worry about. I have never, ever, not once been questioned by anyone over a less than adequate BOL.
And I am also not speaking for HAZMAT. I do not haul HAZMAT!airforcetoo Thanks this. -

Don't worry about it or screw it is not how you teach.
Besides, anyone asks a question on a large forum, of course several people are going to reply.
