The owner/op that I run for just bought a 2012 Peterbilt 386 with a Paccar MX13 engine in it. It's tuned up to 500 he with a 13-speed behind it. The truck has about 515,000 miles on it. My question is, do we have any issues that we need to watch out for, or are we in the clear as far as that goes? How reliable is this engine? What can I expect as far as mpgs go? I've heard about aftertreatment issues, but those usually happened early on. Any help would be greatly appreciated.

I drove one in a 579, with slightly less horsepower and a 10 speed. The only real issue I had out of it was a factory recall for a cam replacement. A bunch of PACCAR engines had it, but by 550k, if it was one of those engines, it would have been replaced. I actually liked the motor. It was quiet, had plenty of power, and was fairly fuel efficient, even pulling a reefer. It didn't do as well on winter blend fuel, but that's typical. I think my truck was a 450 mated to a 10 speed, and 8 mpg wasn't uncommon unless I was out west. Back east, mid 8's when loaded. Not sure if the 500 will get that good, a mechanic told me, the more horses you have, the more you have to feed them...but the 13 speed will help, especially if you run any mountains very often.
