I think usually the front curtain or even the sun visor covers the cam.

As for how they operate, new ones have been installed and I haven't use them much, but they do seem more sensitive. Maybe a sudden wind gust rocking the truck could possibly trigger it?

When I was OTR, I set mine off all the time trying to couple or uncouple trailers on ice. I didn't feel like I was being observed or something and I was never called into safety.

Just sharing my experience.
