Yes we do that was part of the problem, aside from verification of the problem everything is supposed to be locked out. I got my hand slapped awhile back because I was working on something not tagged and locked, and the boss walked in and about lost it, I didn't see the need on it though because the only way to get to it was to move me, breaker was directly in front of me, didn't matter.

 

I wouldn’t want much part in playing with those thick arse wires at big commercial places, that’s into one mistake only territory
