I had to jump in an 06 T800 and back a couple trailers into the dock the other day. At certain points I couldn't hardly see anything in the mirrors because they were vibrating so bad. Been so long since I've sit in a KW I forgot about this issue.... Anyway to get to my point, is there any way to solve this, besides like slipping the clutch, and revving it up a few rpms?? Is something loose or what the heck??
I purchased a w900 just over a year ago and have this issue, just an extra 50 rpm's takes care of it and it stops, next time my ecm is looked at i am going to ask them to boost the idle up.
You might have already tried this, but on a lot of them you can change the idle speed by turning off the cruise switch then use the set / resume button to change it in 10 rpm increments. It will stay that way till from then on.

Really nobody else has ever had this problem?? Every KW I've ever been in has done this......man I don't see how you can put up with this. Major PITA if ya ask me, glad I'm not stuck in one of these trucks daily!!
