    I am not here to hide anything. I have a question I had a Collar Bone surgery in 2009 What forms will I need to give to the doctors when I get to SWIFT. I need to have a Fit For Full Duty is there anything special that needs to be on the form

    Just clearance from your doctor that you can perform full duties. If you have any medications you are currently taking for it, they need to be listed and specifically stated by your doctor that you are safe to operate a heavy truck while taking them.
