Seemed like those guys only ran West Coast back to Michigan and back to West Coast I've never seen their trucks anywhere else

No. The application says "List Any Tickets Or Moving Violations (Accidents) For The Past 3 Years". And that's exactly what you do. Not 3 years and one day! Always just give enough information to cover the question how it is asked, and not a penny more.
No, but Don't get cute either. This is a Federally Regulated industry and the company has to build a driver qualification file on you. Most of this data is available elseware. A lie of omission is still a lie.
