Work history with phone numbers for past 3-10 years depending on company. Some only require 10 year history if it's driving jobs. Some require IRS documents if you were self-employed in any of this time.

Don't go out to the bars while you're going through school if you're at a company school staying with a bunch of other drivers. Many companies have spies in the area bars.

thanks for that input. have quit drinking to keep my blood sugar down and to stay focused on this new venture. would it be easier to get a passport than to request my birth cert.? i was born in california and l.a. county took 3 months to send me one 6 years ago.

I'm surprised they are requiring a birth certificate. I don't remember that when I went to school many years ago. And you would need your birth cert. to get a passport also. You could call the school and see if there is any other documentation you can provide.

My hubby had to have a copy of his birth certificate for both school and the two companies he's worked for. He went to school last year.

If you live in the same town as the office that you get birth certificates from, there should be an option to get it same day for a bit more money.

    Hey BDS,

I had to get a birth cert a while back .. 2 in fact, I keep losing them .. and you can order them online in most states and get then in a week or so. It is costly, $75 or so for mine, and through a third party (thus the high cost). But if you need it you need it. Just Google 'ca birth certificates' .. lots of options.

I didn't have to have my birth certificate for school, but I did have to have it for the fingerprinting, etc. required for the TSA background check for my Hazmat endorsement. If you need Hazmat for a carrier, you'll need your birth certificate. If you go to work with a carrier that does Mexico or Canada, you'll need it to get your passport. Most states offer an online service through which you can expedite getting the birth certificate to you, but of course, it's for a pretty hefty extra fee. In VA, it costs $12 to order a copy by mail, for an extra $50, you can get it by mail in 2 days.

    Good luck!
