Go talk to HR and let them know you forgot. Remember, at the bottom of the app where you sign, it usually says something to the effect "all entries are true to the best of my knowledge". You didn't deliberately leave anything off, you forgot. Alot of carriers are forgiving as long as you 'fess up and correct the information as soon as possible. If you don't correct it, it will look like you're trying to hide something. That will be when you get hit with falsifying an application. I'd get on it tomorrow morning if I was you.

Best thing to do, for sure, is to go online and get a copy of your MVR for as far back as you can. Usually costs less than $10, and you can print it right now. That'll answer the question of whether it shows up.

Then, as brsims says, disclose the non-moving violation to cover yourself.

How was the question asked? Did it say list all moving violations? Take the advice and let them know. You haven't told us what it is so they may be some problems so it's best to tell the truth. If it's for something bad most likely the company will find out anyway and you'll be canned with a bad DAC report saying you're not truthful. Then you become untouchable.
