The USB will plug into your laptop or Desktop.
The Hotspot will allow you to co nect your Tablet/Laptop andyour smartphone at the same time, basically like a mini router.

your smartphone already has that capability. no need to connect to a mini router. LOL.

didn't think of the router possibilities though.

OK after alot of reading I would like to know what I can do with my 3GS AT&T to get wifi in my truck while traveling? I need to have the ability to use my

Ipad or laptop to obtain new loads. I am grandfathered in on the unlimited plan as well. Is there a way to obtain a hot spot using my phone?

Depends on the phone, I have Verizon with a Razzor Maxx phone, heck I can even use my wireless printer from my phone.

ATT knows if you tether with an iDevice. They have taken away people's unlimited data. The best thing to do is buy an unlocked Nexus or similar Android device.
