The person working in your safety department could very well have just skimmed the report, saw 'backing' and 'garage door' and deemed it preventable. Being over-worked like most office workers, he/she could have simply missed the part about 'somebody else lowered the door'. The other thing is, they could also assume you are making up a story to avoid accountability (would you believe people actually do that??). In either case, your plan to make a phone call to clarify things is a good one. If you don't ask the question, the answer is always 'no'.
Yes I suppose that's possible. I still think maverick is a pretty good company and would recommend them. Hopefully I can get this ironed out though.☹️
If it wasn't your fault and you know that's the truth. Do all you can to get it changed or taken off your record.
Talked to safety today and they agreed to remove the preventable! I guess they can be understanding if givin the chance lol.
