The person working in your safety department could very well have just skimmed the report, saw 'backing' and 'garage door' and deemed it preventable. Being over-worked like most office workers, he/she could have simply missed the part about 'somebody else lowered the door'.
The other thing is, they could also assume you are making up a story to avoid accountability (would you believe people actually do that??). In either case, your plan to make a phone call to clarify things is a good one. If you don't ask the question, the answer is always 'no'.
