Through an untold number of inspections I have almost never had an officer even question the absence of such information. I also don't write what I was doing during ON-DUTY in the remarks section and the one time I was asked about it I simply stated that it wasn't required. That was good enough for the officer.

yes city and state is all that is required, and a seasoned officer most times isn't going to push because they know but a newer one seems to need to make a name for them self's

MOST dot officers are pretty easy going, there not going to pin point you for stupid #### unless there the hot shot that thinks they know everything and chances are they dont. just stand up to them and say what you believe is correct and they will say there spheall and they will let you on your way.. remember its just DOT, dont worry about it if you have a valid license and med card with a good truck

With me, it's probably more idyocyncratic than anything else, but I too hated blank spaces. I usually just put the same number of miles for both lines.

Blank spaces just look so unfinished to me. Now, I have go decorate my dashboard with pretty lights....

Already answered here. Team vs single driver. Blanks are OK when they do not pertain to you.

2 requirements, one requirement for a pre-trip "before the vehicle is placed in operation", and one requirement for a written inspection once per day.

Since the "from" and "to" are not required items by 395.8, you can fill them in, or leave them blank, or doodle pictures in them.

I never used to do it either but a (not so) friendly Texas DPS officer did a roadside inspection on me about a year ago and wrote me a "form and manner" log violation. No fine but it is on my CSA under fatigued driving. I started to put where I loaded to where I deliver on the from and to section. Same with BOL/Shipper info and commodity.

I started to argue with this guy that there is no where in the rules that said it was required but then my common sense kicked in when I realized he was gonna write me for something regardless.
