    That's very close to what GM is having all "visitors" fill out. In GM's case it was only 3 questions though, plus contact info.

    Or worse, unloaded.....
    I will not admit to lying on my form for GM. I will say if everyone was truthful at work only about 1/2 the drivers would be let in.
    The last question is the kicker. Cold or flu like symptoms in the last 14 days. Well the weather changed last week here and everyone is coughing because of allergies at least.
