ISX - SCR Efficiency Low 4364

I have 2013 KW T660 with ISX 15 665K miles
My check engine light is on for code 4364 - SCR
Efficiency Low, I’ve replaced so many parts:
DPF filter, inlet NOx sensor, outlet NOx sensor,
DEF dozer, DEF filter, diesel dozer(7th injector),
couple temp sensors and still getting
Fault code 4364....can someone help?
