OK, this is something I know first hand about.
Some states require you to hand deliver a copy of the long form to MVD. AZ is one of them. When I had my physical, I just sent in a copy of the card. Well they sent messages which I did not get and revoked my CDL. I was not driving at the time so I did not know.
I got medical coverage again and got the physical, brought it to MVD and they said I did not have a CDL anymore. WHAT???!!! What do I do now? They said you can apply for a permit and start over. Now keep in mind this is over a year, that is the cut off point. Long story short, the supervisor fixed it for me after some begging and getting them some forms.
So if you medical card is expired for 3 months, get a copy of the long form and get to MVD ASAP. There is a fine for it expiring, mine was $10.
Also make sure to keep that card on you at all times while driving!
Take care, Larz -
Dmv didn't ask for it when I got mine... but it could be different through different states

In Alabama you have to have the long form present when adding Hazmat to a current class A CDL. You have to also have the back ground check done before adding Hazmat. As far as renewing a Class A CDL you just pay the fee and renew it.

Every state is Different. In Ca., if your medical expires, your CMV portion of your drivers license is suspended until you renew and take a copy of the long form to DMV. You will not be penalized for being out of the loop for 3 months. In Az., it's the same except after 1 year of no medical long form on file, your CDL is void and you will have to start over if you want to get back in the loop. In any state, you must have a current medical card to keep your CDL valid. If it expires while you are trucking and you get stopped, you will be put out of service on the spot.

I've never had them ask me in Ohio.. of course I live in a small town, that could make a difference

Why ask on this forum about if you need to get it to the DMV? Got a phone? Call them and ask! That's the FASTEST and easiest way I can think of!

I have a CDL but no medicical card, I dont drive trucks just yet. Do i need to get it asap?

Double check with your MVD, but I would say yes!

Larz is right. Here in AZ. You get a letter from DMV telling you that your physical is about to be out dated. I go to dmv and get there forms. some doctors have there own. I take it to dmv office. They will make copies off your original. Don't let them have the original to keep. out sice of wickenburg a few years ago. they were pulling trucks in for inspection. I got out to get the paper work for the tanker. The driver behind me got arrested. He didn't have a cdl or a health card. He had a load on his rig.
