Hello everyone.I have 2006 Volvo 780 D12D Auto with 1.1 mil .I have the following problem.
I have already changed injector #3 three times.The first two times the new injector lasted around 100k miles.The third time - only 15k miles. If i drive bobtail, no code comes up and no problem with third cylinder.
If i pull an empty dry van the following code MID 128 SID 3 FMI 5 comes up after 35 minutes of driving.
If i drive loaded the same code comes up after 10 minutes .Every time this injector problem started to come up,the timing of the code pop up was the same on all 3 new installed injectors.When i drove to the dealer 180 miles bobtail to change injector,they scanned the truck with their computer and no active code come up.
But i changed the injector anyway and drove 100k more without any problem.Now i am on my 3rd failed injector on cylinder #3 and wondering what to do.
Any ideas?Any advice? Any suggestions?

I'm wondering if the last placed didn't install something correctly....could be something as simple as a loose injector connector, bad injector harness etc. Because it happens under load i would gather its possibly degradation to the harness and your not getting full voltage to injector when it calls for it. Your going to have to take it to a good shop that understands how to do basic electrical really well and verify that everything electrical is good to the injector. It still could be the injector itself has an issue as the cylinder pressures come up but i would start with the basics before anyone throws parts at it.
MID 128 = Engine Control Module
SID 3 = 1/2/3/4/5/6 Unit injectors
FMI 5 = Open circuit in actuator circuit. Short circuit to battery voltage (injector high side). Short circuited injector (injector low voltage side). Short circuit to ground (injector low or high voltage side).
