I'm not familiar with Detroit, but i had same issue with my volvo D13 ( mostly in cold time of the year), moisture inside and throws egr flow codes, could also signal leaking egr cooler, because is located on venturi tube downstream from EGR cooler. But mostly because there's moisture in combustion gases/egr flow , venturi is located in front on engine ,(cooling fan blowing it also), and the sensor sitting on it cools down , water vapors inside condensate into water droplets on cooler sensor surface throwing codes. Volvo undated the venturi tube , from 2012 i believe, comes covered in some kind isolation, to prevent condensation inside sensor.

    ***UPDATE***
Found the cause of the moist build up. Apparently, The mixer pipe, which is the pipe that follows the venturi pipe towards driver side, had soot build up on it causing higher humidity. The original mixer pipe had about 600k miles. Since the mixer pipe was replaced the pulsating/intermittent idle soft noise was gone and the idle is continuously smooth. Venturi pipe was also replaced at the same time which means that the ports of delta pressure were already new or clean along with the mixer pipe. Venturi and mixer are two different pipes. please note.

I did and it solved all the codes. I noticed that the pipe is now coated in a Teflon type of finished unlike the galvanized appearance of the original.
