The only big concern is currently ongoing or chronic psychiatric care, or an involuntary mental health hospitalization. Even for the background check for hazmat or TWIC, a mental health hospitalization is only an issue if it was involuntary.

Even here in 2021 there's still a lot of unjustifiable stigma floating around regarding mental health, and I'd respectfully suggest everyone take some time to reflect on the matter.

Nervous Breakdown is a vague term. Not a medical diagnosis. Depending on the diagnosis, and required meds if any, it may or may not be an issue. I agree too much Stigma. The Stigma only causes people to keep silent. Only bad results come from keeping silent. Medical records are Private. 10 yrs ago, no major meds. It didn’t happen. No ones Business. Most Employers are more interested in getting you qualified, not disqualified. If you’re taking meds, need to find out if they’re disqualifying or not. Less info is better. Especially because of people’s ignorance. It doesn’t matter if you’ve Driven a car recently or not. Unless there’s a medical reason you can’t get a License. Half the world qualifies for some diagnosis. They’re just untreated.

Personal conveyence and the CDL are two seperate items. DOT and hirability dont look at what you do in a car. All they want to know is what your CDL history is. If you dont have one or any in the last 10 years you will be required to have had a refresher course prior to getting hired.
