Do I need to buy a paper packet from somewhere like jjkelller, or will something like this work?

The other things I am unsure of is the driving test, I cannot sign off on that myself can I? If not who do I have sign off on it?
Do I have to list my previous employers if they were not trucking industry related?
Do I have to contact the state to request my driving record?

Have to do everything you would if you hired a driver. And yes, you can road test yourself.

no road test required for many years, just like no test of DOT rules from the little green book anymore.

You just send a letter to old driving jobs and keep a dated copy of them just to prove you sent them out if co will not answer, if you had jobs before you got CDL then you don't have to incl. them if past 3 years, its always funny to see what old driving jobs send back about your greatness at driving a truck?

Annual copy of record, copy of actual D/L, annual convictions of moving violations form. I filled out an actual application, listing previous employment. Past 10 yrs. Sent letters to previous employers. I also keep the annual updates and record along with copy of current cdl, together in the original Drivers file.

When they review a 391 file, and everything is in a logical order, no turning the thing 90 degrees every other page, not having to look thru piles of stuff to find a missing page, they breeze thru it.
Make them work and they will start to nit-pick the completeness of every item, you don't want to slow them down!
J J Keller's package is close to overkill. Search online for a 391 application, print it out and go thru it looking at PART391's section [do get the supervisor 'desk copy' of the FMCSR from JJ; it has the DOT guidance alongside the rules] where it tells you what must be on the qualification application, etc. and you will find most do cover it all.
Haz mat still needs a road test, otherwise you can use the state issued CDL [there is a form for that!] copied in 'lieu of road test'.
Don't forget to sign up for the drug and alcohol database and run your history every year...Last edited: Jun 22, 2021
