I just had the double idler gear replaced on mine. I have 1.3 million miles. Cost me about 16k but had other things replaced while it was apart.

It really depends on the condition of the rest of the rig.
If everything has been maintained well, in good shape, clean and in good condition, with nothing needing major repairs it may make sense to fix it, especially if it has been otherwise reliable.
If it needs a lot of work soon or has been problematic for you then consider a new truck. -
