I listed the medical center where my doctor was at when I last seen them.

I had not seen a doctor in 3 years and had 3 renewals.

You're worried over nothing. Your DOT physical does not ask anything about previous doctors/visits. It does ask about previous ailments, such as, "have you ever had" etc. Unless you had an obvious surgery with an obvious scar, you don't need to mention flu bouts etc. And nowadays, medical records are very private, something like the privacy act. There is no "clearing house" for employers to access every Doctor in the country for any records on any patient.

Don't worry about them checking your "records" if you haven't been to a doctor in years. Just put NONE on the line and if they ask for last time you saw a doctor put 10 years ago? with the question mark!

Me if I wasn't bleeding out of every pore in my body I didn't go see a doctor! Now I wish I had!
