Seems most of the shaking is around 45-65

Like I've mentioned, check u joints first. Seams like there is a bad one. If you step on gas, it gets worst? If so, first one to check is on transmission.

A Freightshaker shaking? What's the issue,,
, okay, sorry, it's just years ago, you tell a boss that, they'd laugh you out of the office. Vibration under acceleration, is almost always the driveline. If it didn't vibrate before the carrier bearing, a 180 off driveshaft could cause that, but more than likely a bad u-joint. How old is the harmonic balancer?
