DOT Physical

    Is it customary for an employer to request the Long Form Physical? I was under the impression that the detailed info that is listed on this form is confidential.

    Yes, it's common, but seems not every carrier requires it. Yes, there is medical information on the form that you may consider confidential, but there is no law against them asking you for it or for you to provide it to them. You don't have to provide it, and they don't have to employ you if you don't. HIPAA does not apply in this circumstance because it's not a doctor disclosing the information.

    Understandable, but why would a prospective employer NEED the info?

    I don't think they do need it. As far as I know, there is no requirement for the carrier to obtain or keep a copy of the long form. They ARE required to obtain a copy of the short form certificate, make sure it's valid, and that the medical examiner is listed on the federal registry. I'm not sure why some feel the need for the long form. Maybe to push CPAPs on their drivers.

    Who gets a short form these days?

    Isn't the employer always required to have updated med and license?

    Employer, also the carrier if you're leased on.

    The employer is only required by FMCSA (49 CFR 391.51(b)(6)(i)) to obtain the Medical Examiner Certificate, which is the short form.

    I've had them ask for the long form. It's not uncommon as far as I know.

    More than one carrier has been slapped with financial loss, and cease and desist from courts about being too nosey in driver's medical history. It's all on the internet.

    If the employer pays for the physical, they basically have the right to a copy. What's on your physical that's a big secret ? Just curious
