the Cam sensor does speed and Top Dead Center
touching the throttle shouldnt be needed and im suprised it helps
if you hit it with Either and it runs im wondering about the fuel pressure being low
and once its running on Either its turning fast enough to build more fuel pressure
and also the ECM might have low battery power while your cranking
make sure the power getting to the ECM is high enough while cranking

I just checked your wiring schematic and if you take your truck side ecm plug out and look #6 should be switched battery power
and #4 should be unswitched battery power
make up a light that draws decent amperage (a driving light) with a pigtail that will plug into the ecm plug nicely and then ground the other side this will load the wires that supply battery power to the ecm
do one wire and then the other
while the light is on wiggle the wires anywhere you can get your hands on them if the light flickers or is dim then the ecm isnt getting enough power to run while your cranking
dont damage the terminals in the ecm plug ........ its best to use an actual ecm terminal im not sure how close you are to a place that would have those .....
Pete Kenworth Cat places like that should have them and they arnt expensive for one or two

    EverLuc Thanks this.

I replaced my starter about 60 days ago. and this morning at 12 am I went to start my truck as it was warming it started to squeek and it was the bendix not disengaging. I turned the truck off and the starter kept going.
    after I disconnected the batteries.
My question is what would cause this and how do I fix it.
    it's a 99 peterbilt 3406e
