Any issue with the EGR? Wondering if that is causing some issues. On my older truck I had a small open in EGR cooler and it seemed to add most nasty stuff to the fuel filter. But that was a Volvo D13 so just throwing out stuff from that engine.
Second thought what RAR's are you running? I believe those engines run better in higher RPMs vs the Volvo Engines.

So its either A i have a lemon engine which is pretty likely considering its history. B. Its a paccar problem and mine is just worse then others. C there is something somewhere stupid i cant find contaminateing the fuel between the piston and the tank. As i said only solution so fat has been more filters.
And for clarity when i say pump i mean entire fuel injector assembly. When that honda hit me chunks of honda and cooling system going bomb mode smashed into that area and broke and bent a lot of stuff around there so they replaced all of them due to one being broken off and unsure if the rest were damaged. -
