I'll quibble with this.

I think it was more to do with reputation than courtesy.

Even in the cities, people existed in neighborhoods were everyone was interconnected. Everyone doesn't know everyone else, but they know someone who knows someone, so following the "golden rule" of "do onto to others" would have a pretty strong feedback.

If Bob acts discourteously and gets told off by someone he doesn't know Bob will shrug it off. But if the next time Bob sees Aunt Sally, she gives him 'what fer', he might think twice about acting the fool.in the future. If Bob then gets ridiculed at the barber shop, and the baker gives him the day old bread, and so on, then Bob will straighten up and fly right.

In the modern world we don't have those connections. Everything is transient. No one works for the same company for 40 years, lives down the street from the parents and has their high school buddies as their bowling team.

For courtesy to go up, we need to be subject to one another.
