If that engine had been submerged in a flood or even driving in high water that is what it could look like too. After the last few big hurricanes that hit Houston and the floods we had seen a bunch of that.

There wasn't any signs that I normally see when water is mixing in the oil. The fill tube and dipstick look normal. The truck had sat overnight and when I pulled the oil plug no water came out. The truck was overhauled about a year ago. I have serviced it for him for years and always use Mobil Delvac right out the gallon jug or 5 gallon bucket. He said he has not added any coolant and it is at the proper level at the moment. I have had other trucks come in where the driver did not service on schedule and never seen anything like this.
I agree that it does look like water in the oil but I don't see the milk look like I would expect. I serviced it and told him that we need to change the oil filters in 2000 miles and do another complete oil change in 5000.

I was thinking that since he has not added coolant that it might be combustion sneaking in somewhere causing soot build up.

Combustion in the engine oil will almost be gritty to the touch. Almost like carbon in the oil. Usually blacker than black. But thick like that.

Thanks I did not know that.
I always expected coolant in the oil to whip it white. That is not true with some of this oil and coolant anymore. I have seen oil that looked good on the dipstick. Let it sit overnight and could drain antifreeze from the bottom of the oil pan.

That brings a memory to mind after what wore out said.
Years ago an employee rebuilt a 3406B. A couple of weeks later it was leaking oil out the front, back and plowing the dipstick out.
Found the screen in the blow by housing on top of the valve cover stopped up with soot. Assumed he missed that. Replaced that and life was good for about two weeks. Then it was doing the same thing. He was checking and found one of the #6 exhaust valves could be moved side to side 1/4" at the top by hand with the keepers still on it. He pulled and I took it to the machine shop. Showed the deal to the man and asked what caused this. " Well could be lots of things. Maybe #6 was not getting lubricated as well as the others. In 5 or 600,000 miles it adds up. " Then I told him you just did this head about 4 weeks ago.
Fixed the head and gave me a head gasket set. That was fun.

CAT had a problem with valve guides for a while some years back. A bridge being out can get a valve guide in a second too. As well as pull the bridge pin, break the pin etc. then hell ensues from there. I had one brought over last year that would loose oil pressure at cruise speed when it finally got down to 30 at cruise he discovered it still had 30 at idle. So an oil filter fixed that he drives it bout 200 miles starts again so I explain when I told him to change filter that was a get it home deal not a fix how did you miss that. I get to digging into it and #5 nozzle didn’t have the white compression seal on it. You could see compression been blowing past it. After finally getting it cleaned up on bottom that took care of it

Well he took the truck and said he was going to drive it until it would not go anymore. Said he wasn't putting another dime into it. I gave him some options (pulling injectors and replacing o-rings), pulling the head to check things out. He was talking about getting in a company truck before this so I guess he will go till it blows. Not sure how everyone else is doing but the container drivers around here are not getting paid for what they do and they are hauling way heavier than what they used to. One of the companies that I work on some of their trucks closed shop last week.

Prolly got the same kinda grime inside it, as is accumulated on the oil pan. Definitely has not been taken good care of.
