My DOT Medical Card is coming up for renewal & I'm worried about passing my physical due to borderline high blood pressure. At home, my BP is in the mid to upper 120’s/70's - so I know that I don’t have high blood pressure. But, every time I go for the DOT physical, my BP jumps 10+ points at the doc’s office = white coat syndrome, and I test at mid-upper 130's/80's and BARELY get my 2 yr card.
But last year, due to a lot of life stresses (deaths in the family & financial) + the white coat syndrome, my BP was in the low 140's & I only got a 1 year/one time Medical card. The DOT doc told me that next year my BP had to be 139/89 or lower in order to get a 1 yr or longer card. Which of course is what I need to keep my job! So, needless to say, I am really worried about getting re-certified & being able to keep driving & support my family.
So, if anybody knows a REALLY, REALLY, SUPER FRIENDLY Dot Doc (you know what I mean) in the Atlanta or N. Georgia area, could you let post it or Direct Message me? Pretty please with sugar on top - I need to keep my job!

Man I feel you. I've been dealing with this since I was a teenager. Its really nothing to worry about. I end up getting a 1 year card every year because of it. I'm on a low dose of bp med and as long as its no more that 159/99 your good to go. This last time I finally had it around 127/82 but still got a 1 year because of the med. What sux is its always normal at home.
I wonder if that Petro up there has a dot doc..they are usually more understanding. Whatever you do dont go to concentra..they act like you are becoming a fighter pilot and will flunk you in a heartbeat.tscottme, Lonesome and bryan21384 Thank this. -
How about this.....just watch your salt and sugar intake between now and then and don't worry about it so much. It doesn't matter how long the card is for, just as long as you can still drive. Don't eat or drink a thing besides water past 7 pm the night before til you after you do the physical. Control what you can. Worrying about what you can't control is mental masturbation.

One thing you can do while waiting in the waiting room is slow your breathing. Take long deep breaths in thru your nose and out your mouth. Controlling your breath which will slow your heartbeat and calm you down... Do this all the way until they put the cuff on your arm. It has helped me. I always would test high until i had a doctor give me a couple minutes to rest and do these breathing excersizes and it dropped my blood pressure significantly

My understanding was that if you tested over 139/89, but lower than 159/99 - they would give you a medical card for 1 yr only & that the next year, you had to have BP under 139/89. Where are you getting 1 yr cards year after year with BP over 139/89? -- I realize that's a sensitive subject, you could direct message me

I've been in the 159/99 range for 2 years or longer before and nothing was said...I still got a one year card each time. Here the past 2 years though I've managed to keep it under 140/90. At least if you are on bp med it will help you control it. Very likely nothing at all is wrong with you, its just anxiety.. ..it sux. I usually go to an urgent care, like family med care. ..or find a chiropractor thats does them. I used to go to one regularly but he retired. Wjere are you going?Last edited: Nov 12, 2025

I've been going to Piedmont Urgent Care here in Atlanta. They seem VERY strict on everything about the exam. Needless to say, I won't be going back to them for this physical.

Go to this site and find yourself a chriopractor or doctor in a smaller office to do the exam thru. Enter your zipcode or whatever it asks it will show u a map of all the people around you that are certified to do dot exams
