No oil in the hub. Wheels dont fly off for no reason.

The biggest problems with wheels coming off that prompted the whole law, were dump trucks with guys names that sounded like they were in the Godfather movie. So crack all you want about this drivers name, it does not mean squat but go to demonstrate your ignorance.

But..... on to the issue of the original report. I have heard the biggest issue is not the lug nuts but rather the hubs run out of oil and cause it. Has anybody else heard this or experienced this?

Well to further DEMONSTRATE my IGNORANCE yes I have heard of the loss of hub oil causing a run away Wheel & Hub as well as witnessed it on a container chassis of 1 of CANADAS famous Can Haulers ( GARFIELD ) a few years back near Watertown NY.

You must not be old enough to remember the guys getting caught with 6 CVOR's in Maple when all those trucks were losing tires. But carry on.

Depends on, why the wheel came off? If it was a hub failiure due metal fatique then usually there there is a small fine associated with the incedent.
If the wheel came off because of rusted studs, loose bolts, cracks in rim then the fine is $50,000 + + + .

This is what I was told by the MTO.
