how intence is the DOT Physical I had a surgery in 2009 while I am on active duty navy to repair my colar bone they put in a plate and 10 screws will i need any paper work before i start training at Prime. Will I need a paperwork from my Surgon stating that I have no limitation

You'll want to speak with your recruiter for specific details, but the DOT physical is not too intense. The vast majority of people who fail it is due to high blood pressure, not past physical injury.

Yes, if you answer honestly on your medical form... you will need documentation and clearances from your doctor.
I believe the questions asks about surgery within the last 3-5 years. -

Better safe than sorry... While some guys in my class got their stuff straightend out by fax from thier Dr... other guys got sent home to get what they needed from the Dr and then rescheduled to come back.
Some guys straight up failed and got sent home. -
so a letter from the doc stating fit full full duty with no limitation will be fime or will i need anything else

It's hard for us drivers to tell you with 100% certainty that you'll be fine with the letter. I'd think you would be, but we don't have any kind of "inside" information. We simply speak from our experiences going through orientation.
I'd talk with your doctor about this, as well as your recruiter, to be sure you have all the documentation that you could possibly need. Bottom line, if you think you might need it, bring it with you. A sheet of paper doesn't take up much room. It's better to have too much information than not enough. Like U2 said, some people get sent home from orientation, not because they necessarily fail the medical, but because they are unable to acquire the proper paperwork from Springfield. If you have a known medical issue that you think might need some clarification, bring as much documentation and paperwork on it as possible, just to be safe. -

During the Telephone Interview the recruiter asked my height and weight. He said it is to check my BMI........ I responded 5'10" 250 lbs. he replied that my BMI calculation was 36% and I was OK.
Prime has a policy to screen drivers with Sleep Apnea Syndrome. Applicants whose Body Mass Index is 39% or Higher will have to under go clinical testing for the condition.
According to FitnessMagazine.com 25 to 29.9 is Overweight; 30 to 39 is Obese; 40 to 200 is Extremely Obese.
Time too put down those Big Gulp and Super Sized Big Mac Combos!
