Wher to get a DOT physical long form.

    Thanks. Do I need to carry the original with me in the truck or should I keep that in a safe place and carry the copy. All Ive ever had before is the fancy carbon coppy type before.

    If you're paying for it, I would keep the original. My company asks for it, but I always give them a copy. I'm sure it doesn't matter. Just keep the long form in the truck just in case some slim chance some state asks for a supporting document. There's a few post here where some drivers had to show it.

    In Ca. DMV use to have the long forms, not sure if they still do.

    Keller (the logbook people) sells a generic long form package at almost every truckstop.

    I gotta ask this ?, since the nurse does BP check, weight, height and the initial greet and wee-wee test, and the Doc really only does the heart, cough thing plus the whisper maybe, why not go to the clinic ?

    Yup I went to a pilot today and their they were. It was $4.99. I paid the 5 bucks because it comes with a laminate for the wallet card. I don't know where to get things laminated and for 5 bucks its convenient. Thanks for all the help. First time in 16 years the doc. doing it didn't have the paper work. I bet he didn't drop the price 5 bucks when he stopped getting the forms.
