I'm still around and still wanting to become an owner/operator! I have talked to several companies lately and can get several rates. One company is very serious and is offering $1.13CAD per mile for loads under 52,000lbs, and $1.17CAD per mile for loads over 52,000lbs, with all O/O averaging 10,000 miles or more per month.
How do I put this into a business plan for a banker? I've looked everywhere online for business plan templates, but cannot find anything where a owner/operator hires his truck under the authority of another company.

I would say you need to show the banker dude some facts, such as, initial cost of your truck, fuel cost (current and projected), fixed costs, such as registration, insurance etc. How you plan to pay for repairs if needed, how much you have in savings right now for back-up, how much you need to pay expenses monthly, such as food, rent, utilities etc. and the company you will be leased to with proof. And your experience level will help too.

You will need to add facts such as fuel surcharge in the offerings etc. Also whether you will be solo or a team. Facts like that go a very long way.

Oh I know what goes into a business plan. Any and all business plans I can find for a trucking business is for someone who goes into the business with their own authority and finds there own loads, their own customers, etc. I'm basically looking for someone who has come up with a business plan who did or had planned to hire right on to another company, and see exactly how they wrote up their business plan.

You have to do what I did with mine. Alter one of the existing trucking ones to suit your situation. Use it as a guide only.
I ran numbers on mine being conservative on mileage income and liberal on the expenses side. Knowing full well that if it will not work in the spreadsheet then it will not work in life.
At only $1.13 I sure hope you are getting a good fuel surcharge as well as not being deducted every pay period for the plates and insurance. You really need a copy of the lease contract in order to make sure you can account for every expense and piece of income. The more you can get, the better off you are. -

Fuel surcharge is 90% of average. Insurance is my own responsibility.

Does that rate include FSC? With all the talk in the news over the years about how bad fuel has hit trucking companies I would add a section on FSC - how it is calculated, history, etc. That is by far going to be the biggest factor in your expenses that you have no control over. So by point that out and showing how it is controlled you avoid that objection.
